2. Informative Copy of Transcript of Records for evaluation purposes
3. Special Order / Certification for Exemption of Special Order
4. Black and White 2"x2" recent photo (2 copies)
5. Original copy of Form 1A (Birth Certificate)
6. Receipt for permit fee (Certificate of Eligibility for Admission into the Law Course)
7. Request Slip/Clearance Slip for TOR (required in the issuance of CEL)
8. Marriage Contract for married female students
9. Thick long brown envelope
