  2. Entrance Examination Fee

  3. Form 138 (High School Card) or Form 137-A
  4. Transcript of Records (Informative Copy)
    for second courser and shiftees
  5. Certificate of Good Moral Character duly signed by the High School Principal/Dean
  6. Birth Certificate from Local Civil Registrar
  7. Medical & Dental Certificate
    (Secure form at the CON Office)
  8. Certification from HS Principal that
    the graduate belongs to the upper 40%
    of the graduating class (Secure form at the CON Office)
  9. Whole body post card picture
    (colored, close up & recent photo)
  10. Police Clearance

  12. Marriage Contract (for married applicants)
